Fitness Questionare

  • List any hobbies here
  • List long term and short term goals
  • Please make sure to be fairly inactive for at least 5 min prior to take the measurement. Place two fingers on your wrist. Find a pulsation and count the number of beat in 1 minute.
  • Perform body weight squats for 1 minute and as above take your heart rate for one minute.
  • Write in the best of your knowledge a day type of all the food and fluids consumed in a day.
  • Please upload a current picture in gym clothes.
